How to Get a MED Badge in Colorado
Step 1 – Download The MED Badge Application:
Download application from HERE. Fill out and print the application.
Step 2 – Email appointment Submission:
Email submission for appointment to the Colorado Department of Revenue here: dor_medoccupational_appts@state.co.us.

5. Day (M-F) and time preference (a.m. or p.m.):
Step 3 – Background Check:
After you have received an email verifying the appointment and time, you will take your printed application to MED headquarters for a background check. A fee of $75 is requested upon completion of the application and approval.
